Senior Living Company Launches Intake Center to ‘Streamline’ Coordinated Patient Care

StoneGate Senior Living, which develops, owns, and operates a number of senior care and housing properties in Texas, Colorado, and Oklahoma, has launched a centralized referral system to coordinate patient care between Dallas-Fort Worth hospitals and the company’s post-acute-care communities here.

StoneGate’s new Central Intake Center (or CIC), located at the company’s headquarters in Lewisville, is essentially a call center staffed with six employees and a registered nurse. The CIC offers round-the-clock service to the medical care community, connecting acute-care providers looking to discharge patients from their facilities with 10 of StoneGate’s DFW properties.

While providers will be able to phone the center 24/7 at 833-392-3648, CIC staffers also will monitor existing software systems to learn about discharged patients in need of transitional care. StoneGate says the CIC team members will then confirm the plan of care, gather clinical and payer information, secure proper authorization, and place the patient in a post-acute setting most appropriate for his or her needs.

“This streamlines the process, and that’s what the hospitals want,” said John F. Tyler, president and CEO of StoneGate Senior Living.

The  CIC service will coordinate referral to StoneGate facilities in DFW offering the likes of inpatient rehabilitation, assisted living, memory care, respite care, and skilled nursing and rehab services. Over time, Tyler says, the company could hire as many as 20 call center employees, plus three or four more nurses, as it expands the CIC to serve acute-care providers at StoneGate properties in Colorado and Oklahoma as well.
